We are a privately owned Family Medicine office.  Our Board Certified Family Physicians are here to help you manage your health.

We are currently accepting new patients and offer same day appointments.  To schedule, please call our office at 417-624-4060. 

For established patients, we offer online office visits. To start an online office visit, please go to www.relayhealth.com. We are sorry, but at this time insurance is not reimbursing for this service.  There is a $30.00 charge.  If you need a receipt to turn in for your flex plan, please contact the office, and we can provide one for you.  

We also have a patient portal available, whereby you can access your chart directly.  To sign up, please email us at family@4state.com .
